Once you submit your Screening Authorization form to info@tkidst.com, you will receive a welcome email from notifications@fusionwebclinic.com with instructions to sign in to our web portal where you can conveniently review your invoice for your child’s Developmental Screening and/or Vision and Hearing Screening, pay online and print a receipt.
